Services:

Tandem Residential provides TBI/CADI waiver services and in-home family support/respite services to persons with brain injuries or related conditions under the Medicaid waiver.

Residential Services - 24 hour supervision:
This setting generally involves a shared housing situation with staff present at all times. Staff-to-consumer ratio is based on individual needs and includes either awake or asleep overnight staff as needed.

Residential Services – 24 hour available supervision:
This situation typically involves 2-4 consumers sharing a house or apartment, with live-in staff to provided emergency services during overnight hours. Also included are daytime staff hours, arranged according to each individual’s needs.

Independent Living Skills (ILS) Services:
Providing skill training in an environment where skill will be used to promote independence in areas of relationships, leisure recreation, daily living skills and communication.

Person Centered Planning:
Development of a person centered support plan for individual. Each person is looked at from the aspect of his or her needs, preferences, hopes, rituals and dreams. This process assists the individual identify and build a network of support that will assist with planning and implementation of the plan.

Behavioral Services:
Tandem recognizes that individuals are unique and is able to provide behavior management plans to help individuals succeed. Staff are trained in behavior management plans to help individuals if needed.

Tandem Residential also has a 24-hour emergency cell phone system.

We understand that brain injuries affect individuals in different ways. We will team with individuals to provide customized services to achieve individual goals. Services provided are according to individual needs and can include: medical management and administration, meal planning and preparation, funds management, medical appointment management, transportation to appointments and various activities.
